Application Forms

Save yourself some time - Print and fill out the appropriate form below, then bring it with you to the shelter or scan and email it to adoptions@buttehumane.org. All adoptions are subject to the approval of Butte Humane Society staff.

If you live outside the Chico area and wish to place a temporary hold on a certain animal until you can get here, please fax your application to 530-343-3734. Be sure to include the name and case number of the animal you wish to adopt. A hold will not be placed on an animal unless your application is approved. Please contact us to verify that we received your application and whether it is approved.

Please note: if you rent we will require landlord authorization before approving the adoption. If you currently own one or more dogs and wish to adopt another, we will require you to bring in your dog(s) to meet and ensure compatibility.
